Health Care Service Provider (HCSP) receives the "24 Hour Weekend On-Call" one-time charge to wear pager, answer calls and remain within 30 minutes of the facility during any Weekend 24 hour period. If a HCSP must report to facility and/or see patients, the normal hourly rate (except CRNAs) applies for the actual hours present, with a 1-hour minimum. Weekend On-Call period is generally 8:00am ­ 8:00am. "Weekend 24 Hour On-Call" one time charge equals 3 hours at the Hourly Rate for all HCSPs, except CRNAs and Anesthesiologists, who get 4 hours at the Hourly Rate.

Holiday Pay: All HCSPs required to work during the following 10 federal holidays will be compensated at the usual hourly rate: New Years Day, Martin Luther King's Birthday, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran's Day, Thanksgiving Day, and Christmas Day.

All CRNAs required to work during the following 10 federal holidays will be
compensated at 1.8 times the hourly rate: New Years Day, Martin Luther King's Birthday, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran's Day, Thanksgiving Day, and Christmas Day



CRNA Over Time Pay: All CRNAs required to work beyond 40 hours per week or 8 hours per day are compensated at 1.3 times the usual hourly rate. Work Week: Sunday thru Saturday.

Minimum Qualifications of Health Care Service Providers (HCSPs):

Physicians: All physicians will possess a MD or DO degree, have at least one current unrestricted Medical License issued by one of the U.S. States, Territories or Commonwealths, and will have finished residency in their specialty as approved by the Accreditation Council of Graduate Medical Education (ACGME). All physicians will be Board Certified or Board Eligible by their specialty American or Osteopathic Board. All Foreign Medical Graduates will be certified by the Education Commission for Foreign Medical Graduates (ECFMG) and be eligible to practice in the United States.

Registered Nurse-Anesthetist (CRNA): All Registered Nurse-Anesthetists (CRNAs) will be certified by the American Association of Nurse Anesthetists (AANA) and possess a current unrestricted license in one of the U.S. States, Territories or Commonwealths. Experience: Minimum twelve (12) months experience in the past 36 months. Degrees ­ ADRN, BSN, MSN, APN, and Diploma Graduate Nurses.

Physician Assistants: All Physician Assistants will be certified by the National
Commission on Certification of Physician Assistants (NCCPA) and possess a current unrestricted license in one of the U.S. States, Territories or Commonwealths. Note: All HCSPs shall meet or exceed all qualifications as set forth in the FSS Contract under the Scope of Work section.
